The Old Man Plaza Trail is located off of exit 34B in Franconia Notch State Park, you will find signage that takes you to the trailhead. There are two separate parking lots for the trailhead, one that is located up the hill at the Old Man of the Mountain Museum, and the other parking lot being located down a side road down the hill from the Museum (this is where all the accessible parking is located). From the accessible parking lot the trail takes you on a fairly flat path that has many different plaques and signs that describe different facts about the area, and what lives in it. The trail continues on to a large plaza area on the shore of Profile Lake, where there are informational signs about the Old Man of the Mountain. After the plaza there are a few public binoculars stands that you can use to see where the Old Man used to be located on the side of the mountain.
